The Book, Italian Genealogical Records by Trafford Cole has the addresses for the area you need to write to.  It is to the state of the area, in my case it was Di Stato Di Sciacca.

This is the letter I wrote:

Per favore, mi potrebbe spedire I certicati di Nascita di Rosa/Rosina Gelo, nato il 12 ottobre, 1887.  Se possible per favore padre di Rosina’s I certicate di nascita di Carmello Gelo.  Padre matrimonie di Francesca LaBarbera.  Se questi certificati includono anche I nomi del loro genitori, se per cortesia, Potrebbe inivarmi anche I loro.

Prego se ci è della spesa lo ha lasciato sapere. Inoltre se la risposta è negativa lo lasciasse sappia.  Debba trovare dove la famiglia è stata sopportata per ricerca per genealogy.

Where did you write for the military document?  Was it for someone from Termini?  I would love to see documents from any of my ancestors from the military in Sicily.

With a change of times, I have heard to include email address.  I have 
gotten an email answer for one of my request.  The request was to tell me to 
send said amount of money to address enclosed.  Got a military document for 
this request.

  I have done this about 10 years ago by regular mail, short request in 
Italian...I included a small amount of cash, I think about $5.00...I got a 
reply with requested information about 6-8 months later....I don't know if 
email would work.......
