There is no form at the moment.  I need to redo it.

The prices are as follows:

For any entry that has a digital image number, the prices are as follows:

$2 each copy - Births, marriages, deaths, Allegati births, allegati 
promessa or allegati death
Ask for price of allegati matrimonio because it will depend on how many 
pages are in the allegati for the marriage

For any entry that does NOT have a digital image number, the cost will 
be $5 per document.

For lookups in films that have been digitally finished but not indexed 
into the database - this constitutes research.  Ask me for prices
